Trying to connect an iPhone to my laptop icam, questions

I am trying to connect my camera to an iPhone, the phone is not near my computer (not sure if that matters).  I also have a PC.

When trying to log in with the iPhone it is asking for the IP or host and Port.
Were would I find this info so that the phone can connect to my camera?

Are you attempting to connect using iCam on your iPhone and the iCamSource on your computer?
