Same question.

I'm trying to configure a Grandstream GXV3500 encoder/server.

Have you guys tried either of these links to find the correct URL?

No, but here is additional information.

I was able to see the video in iCamSource, iCam on my iPod and Mac Firefox by first setting my Grandstream GXV3500 encoder to MJPEG video and then entering the following URL into the browser:
http://<IPAddress>:<port>/goform/stream?cmd=get&channel=0
(e.g. http://192.168.1.65:8090/goform/stream?cmd=get&channel=0)
Hope that helps.

However I could not get H.264 to work at all in iCamSource or in Mac Firefox. And most unfortunately, the GXV3500 encodes MJPEG at only half the resolution of  H.264 and I could never get the audio to work, so I'm returning the unit.

If anyone knows of a reliable IP encoder/server for a Mac/iCam environment, please let me know.

iCam does not support H.264. Hopefully they'll add support for it eventually. As far as the audio goes, it's only supported for a few IP cameras. Good luck in your search.
